Preventive Action

Risk & Failure Analysis
mechanicalelectricalprocess

Definition

An action taken to eliminate the potential cause of a nonconformity or failure that has not yet occurred but is anticipated based on risk assessment, trend analysis, or lessons learned from similar equipment or industry experience. Preventive actions are proactive measures designed to prevent problems before they manifest.

Context

Preventive actions arise from FMEA analysis (high-RPN failure modes that have not yet occurred), industry alerts, lessons learned, and RBI assessments. They are tracked alongside corrective actions in Reliatic's action management module but are distinguished by their proactive nature.

Examples

  1. Preventive action: Apply thermal spray coating to all carbon steel elbows in the CO2 injection system before erosion-corrosion causes measurable wall loss — based on lessons learned from a sister facility.
  2. Preventive action: Increase vibration monitoring frequency on all pumps approaching 75% of design life — based on FMEA identification of age-related bearing degradation as a credible failure mode.

Applicable Standards

  • ISO 9001:2015
  • IEC 60300-3-11

Related Terms

Corrective Action

An action taken to eliminate the root cause of a detected nonconformity, failure, or undesirable situation to prevent its recurrence. Corrective actions address why something went wrong, not just the immediate symptoms. They are distinguished from immediate actions (containment) and preventive actions (proactive measures for potential future problems).

Failure Mode and Effects Analysis(FMEA)

A systematic, proactive methodology for evaluating a process, product, or design to identify all possible failure modes, determine their effects on system performance, rank them by risk priority, and recommend actions to reduce or eliminate the highest risks. FMEA examines each component's potential failure modes, their causes, effects at local/system/end levels, current controls, and risk priority numbers.

Mitigation Action

An action specifically designed to reduce the probability or consequence of an identified risk. Mitigation actions differ from corrective actions (which address root causes of occurred failures) in that they proactively reduce risk for potential future events. They are tracked in the risk register and linked to the specific risk they address.

Failure Reporting, Analysis, and Corrective Action System(FRACAS)

A closed-loop process for systematically reporting failures, analyzing their root causes, implementing corrective actions, and verifying that those actions effectively prevent recurrence. FRACAS captures failure data, triggers investigation based on severity, tracks corrective action implementation, and feeds lessons learned back into the design, maintenance, and operating procedures.
